Confidentiality and Terms of Agreement
  • I give Student Accessibility Services staff permission to access my academic record for verification and approval purposes, and I give my consent for information to be exchanged for those purposes.
  • I understand that any information about any student that I acquire in the course of being a student note taker is considered confidential and shall not be disclosed to anyone, except the Student Accessibility Services staff and/or their immediate supervisor.
  • I give my consent to the Student Accessibility Services staff to release my name and email address, provided on this application, to students requesting a student note taker.
  • I understand that I am applying for both the peer note sharer and professional note taking role. I acknowledge that SAS will always indicate if the position is for a peer note sharer or a professional note taker and that my response will not affect my eligibility for other courses.
